The $3.3B deal closed in August 2025, a week after a settlement with the DOJ. The settlement required divesting 152 home health, 11 hospice and 1 palliative care locations in 19 states to BrightSpring, Pennant or another approved buyer. UnitedHealth bought LHC Group in February 2023. In a 2023 proxy, Optum said it intended to keep Amedisys's presence in Baton Rouge.

Ownership changed in 2026. DaVita's 10-Q says its $200M non-controlling minority stake closed July 20, 2026. On its Q4 2025 earnings call, DaVita described the deal as its minority investment alongside a majority investment from Ares funds. Mertz Taggart says Blue Wolf Capital held Elara for about eight years before this deal. The scale figures come from Elara's Feb 2, 2026 release.

As of the June 30, 2026 10-Q, the last 3 of the 110 branches were expected to close in 2026. BrightSpring completed no acquisitions in the first half of 2026.

Hospice News (Sep 2026) says Compassus serves 33 states; the Oregon order says 30.

A Delaware court's constructive trust lets Encompass Health and Enhabit split 43% of VitalCaring's ongoing profits and exit proceeds. An early-2026 Enhabit SEC exhibit says three individual defendants settled for $43.1M, and that the order against Vistria, Nautic and VitalCaring's holding company is unaffected. In January 2025, Amedisys halted a 2024 agreement to sell more than 100 locations to VitalCaring.

Will a private-equity platform buy a single, owner-run agency?

Yes. BPEA describes LiveWell Partners as focused on buying individually owned, mostly Medicare skilled home health locations, and LiveWell's website asks owners who want to sell to contact it. Mertz Taggart counted six home health transactions in Q1 2026, and five were add-ons to sponsor-backed platforms. Small single-market examples include Pennant's purchase of Healing Hearts Home Health in Gillette, Wyoming (September 2025) and Creach Family Holdings' purchase of Freudenthal Home-Based Healthcare in St. Joseph, Missouri (July 2025).

Is the home health M&A market slowing down?

Deal counts fell in 2026. Mertz Taggart's Q2 2026 report counts 16 closed home-based care transactions in Q2 2026, down from 29 in Q2 2025, and six of those were home health deals. The firm says fraud takedowns, the hospice 36-month rule, the enrollment moratorium and enhanced oversight are making deals harder to close. MedPAC says home health M&A peaked in 2021 and 2022 and slowed afterward.

What happened to the agencies UnitedHealth had to sell when it bought Amedisys?

To settle with the DOJ, UnitedHealth and Amedisys had to divest 152 home health, 11 hospice and 1 palliative care locations in 19 states. The Pennant Group bought 54 locations in Tennessee, Georgia and Alabama and closed on October 1, 2025. BrightSpring Health Services agreed to buy 110 branches in 18 states. It closed on 103 of them on December 1, 2025 and 4 more on December 31, 2025. As of mid-2026, it expected the last 3 to close during 2026.

Are hospitals and health systems selling their home health agencies?

Many are moving them into joint ventures or selling them. Compassus formed home health and hospice joint ventures with Bon Secours Mercy Health and OhioHealth in 2024. It also bought a 50% interest in Providence's home health and hospice business, closing in March and October 2025. Pennant took majority ownership of John Muir Health's home health business in January 2024. Baptist Health put its home health business into a 50/50 joint venture with Alternate Solutions Health Network in March 2025. HCA Healthcare closed the sale of 31 home health and hospice locations in eight states to Deaconess Associations, a Cincinnati nonprofit, in early October 2026.

The check pass read 49 pages in full and used excerpts from 32 Parallel searches. For every platform I looked for a 2025-2026 sale, recapitalization or ownership change before naming the owner. I excluded pediatric private-duty nursing, non-medical personal care and hospice-only deals. I also left out not-for-profit acquirers and any acquirer whose ownership could not be sourced. \"Strategic / family-owned\" also covers privately held operators with no sponsor behind them. Health-system joint ventures are listed as deals only where the platform took a stake or management role.
